If you have not completed the health check forms, please kindly complete the below documents:

ISHCMC Annual Medical Questionnaire Purpose: Complete the form once a year so we have the Important for your child's safety on campus and so they can attend trips off-campus

• This form is accessible through the PowerSchool parent portal.

• Includes permissions for medication administration and activity restrictions.

Emergency Release of Medical Information Form (ROMIF) Purpose: Facilitates timely and appropriate care for your child.

• This form is accessible through the PowerSchool parent portal.

• Ensures essential medical details are available in emergencies.

Government Physical Form Purpose: Requirement by DoET and local health authorities

• Please take the form to your healthcare provider for completion.

• Please submit the completed form and hand the form to your campus Wellness Center
